Tracheostomy Care
A tracheostomy is a surgical opening in the trachea (windpipe) to make breathing easier. The opening is called a stoma. A tracheotomy tube is inserted in it to keep it open. At Bridgewater,we can assist the tracheostomy patient in understanding how to care for and adapt to the tracheostomy tube and site. At times, due to weakness, neuropsychiatric or physical problems (arthritis), the patient may not be able or competent to handle this situation. Wound Care
Specialized wound care programs are available for severe decubiti and other lesions, or post-surgically. An individualized care program addresses specialized medical, nursing, nutritional and pain management needs. Specialized supplies, treatments and equipment are utilized to enhance healing and comfort. Our staff have an enviable record in infection-prevention, healing of decubiti and promotion of skin integrity with even the most compromised patients.
Intravenous Therapy
We offer on-site intravenous antibiotic, hydration and pain therapy, which results in shorter and fewer hospital stays, better continuity of care and enhances quality of life.
Trans Parenteral Nutrition (TPN)
Trans Parenteral Nutrition (TPN) is provided under the direct supervision of nutritionists and pharmacists. Feeding solutions are administered in accord with each patient’s nutritional and diagnostic parameters.
Peritoneal Dialysis
Peritoneal Dialysis is provided at the facility using the latest equipment and techniques, coordinated by their Nephrologist. The risk of complications (infection) is minimized, and the resident can attain maximal comfort and maintain regular activities.
Hemodialysis
Hemodialysis can be provided at nearby dialysis centers by arrangement. The facility monitors all aspects of resident care with special emphasis on nutrition.
Comatose Care
We care for patients in a comatose or semi comatose state. Our physical therapists work with the patient on a daily basis or as needed.
Bariatric Care
We provide comprehensive services for those requiring bariatric care. Our team will design a program to meet each individual's, needs from dietary
to physical and occupational therapies.
Comprehensive Care Planning
Upon admission, the treatment team evaluates the resident’s condition and needs, and devises a Comprehensive Care Plan detailing each discipline’s interventions to meet the resident’s holistic needs. The goal is to promote the resident’s health and function at the maximum attainable levels. The team meets quarterly and upon any significant change in the residents’ status. The resident and /or their family are invited to attend these meetings to take part in the comprehensive care plan.
